Vial of Life

The Vial of Life (VoL) service is provided by Fairfield Glade Resident Services. It gives first responders and hospital ER personnel your critical medical and contact information, which could help save your life in the event of an emergency. The Cumberland County VoL service was developed in 2008 by a team of organizations including the Sheriff’s Dept., EMS and 911, Cumberland Medical Center, and FGRS.

How It Works:

  • The VoL kits are free and available to all interested county residents.

  • The VoL program is provided in convenient kits.

  • You are responsible for filling out the forms for medical data and medications. As your medical needs change, you can easily update your saved forms and make sure your VoL information is current. Download our Emergency Medical Information Forms, save to your computer, and enter your medical and medications data, which are used by Emergency Responders and/or medical facilities.

  • FGRS offers a 4x6 yellow vinyl magnet sleeve for your home in which to store forms. These can be attached to the refrigerator (or other nearby place). Click HERE to download your medical information form. 

  • Bag tags are available for golf or tennis bags, along with wallet kits.  Click HERE to download your medical information form.
